The Alliance HealthCard program is offered to you by Alliance HealthCard, Inc. The following Terms and Conditions govern your participation in the program. In these Terms and Conditions, "you" and "your" mean each person who has purchased an Alliance HealthCard Gold Card or Platinum Card membership.
Description of Benefits. You and those members of your immediate family that reside in your household may participate in the program. As a participant in the program you are entitled to receive discounts and other savings on certain healthcare-related products and services at various provider locations. The program benefits are detailed in the member guide and are subject to change, modification, or substitution without notice. In order to receive the membership benefits after you have enrolled, you are required to present your membership card at the time any purchase of goods or services is made. You agree that the program is not an insurance plan and you are solely responsible for payment at the time of purchase of the established price for the prescription medications and healthcare services you purchase under the program.
Membership Term and Cancellation. You must pay the established membership fee when you enroll in order to enjoy the privileges of the Alliance HealthCard membership. You may cancel your membership at any time. If you wish to cancel your membership, you may do so in writing. Mail cancellation letter and your membership card to: Alliance HealthCard, Inc., 4929 W. Royal Lane, Suite 200, Irving, TX 75063. Your membership will terminate effective as of the date your notice of cancellation is received. If you cancel your membership within thirty (30) days of your initial enrollment, Alliance HealthCard will reimburse your membership fee within thirty (30) days. If you cancel your membership after the initial thirty (30) day period, there will be no refund of any fees you paid. Alliance HealthCard may cancel your membership if the program is discontinued for any reason and would, in that event, provide you with at least thirty (30) days' written notice of the cancellation at the address you have provided to us. However if you fail to pay membership fees, as required, or if you violate any of these Terms and Conditions, Alliance HealthCard reserves the right to cancel your membership immediately.
Use of Membership. Your membership benefits are not transferable or assignable without our express written consent. You agree to use your membership only for your own personal use and for immediate family members living in your household. You and Alliance HealthCard agree that there are no additional third-party beneficiaries under the program.
No Change in Status. You understand that your prescription medications are dispensed only in accordance with the order of your physician or other health care provider. Membership in this program does not alter or affect your ability to obtain prescription medication or your responsibility to use prescribed medication in accordance with the directions of your physician or other health care provider and pharmacist.
Confidentiality. Your pharmacy ensures that customer prescription
information remains strictly confidential. Pharmacies work with agents and affiliates,
also bound to strict confidentiality, to perform certain functions under this program.
Customer service for the program will be handled by Alliance HealthCard. As an extra
service to you, as a part of your enrollment in this program, we may send e-mails,
direct mailings, or telephone you with offers or information that may be of interest
to you. For these purposes, we may use outside processing companies as an agent
but they are also bound to strict confidentiality. Your privacy is of utmost importance
to us. We will never sell or lease our membership list.
